Interactive Forms for the New PHS 398
The PHS 398 is the standard grant application form used in most NIH grants submitted through RFMH. Below are the forms in interactive Acrobat (PDF) format. The Acrobat Reader is free. With the Reader, you can enter information on the forms and print them out--BUT YOU CANNOT SAVE THE FORMS. To save the forms, you have to have the full Acrobat program.
We are looking into the possibility of obtaining a site license for RFMH and each interested department would then receive one copy of the full program. If your department would be interested in receiving this, please e-mail us.
